USEFUL INFORMATION
These Letters are based on Federal Laws and State Laws. I am a Licensed Clinician and will only approve letters based on truth and integrity.
I am Licensed in 7 states: IL, IN, AZ, IA, TN, MS, NE
Emotional Support Animals can be a cat/kitten, dog/puppy or rabbit that will live with you in a residential rental property.
Psychiatric Service Dog can only be a puppy/dog that will travel on an airplane with you. There are weight limits for pets, please check with your preferred Air Carrier.
All pets are required to have documentation of recent Veterinary visit for housing and travel.
These letters are for children age 5 +— adults age
These letters expire 1 year after approval date.

Prior to our session, you will email: proof of identity, proof of a DSM 5 Mental Health Diagnosis, ESA referral from your doctor/therapist, complete the documents that will be emailed to you..
We will meet via a telehealth call or video on the scheduled date.
If you are approved for the letter, you will receive the letter within 24 hours. If you are denied, you will receive a refund. The letter is written on my letterhead, personalized to you, your pet, state and my clinical license information.
You will receive a Treatment plan with your letter.
We stay connected for the year, you will be scheduled for 2 follow up sessions.
